Welcome to the National Tuberculosis Controllers Association (NTCA) membership application page. Membership in this organization provides an opportunity for you to share in the benefits of collaborating with and learning from your colleagues in TB prevention and control.

To join the NTCA or to renew your membership, please complete the NTCA’s membership application below. If you have any questions, call 1-877-503-0806 (toll free). Membership dues are due at the beginning of each successive year.

Membership Application and Payment Methods
We have new online options to make it easier to join NTCA or to renew your membership. You can submit your membership application online or by hard copy sent through fax or mail. Membership dues can be paid online, by telephone or by mail.

  • To apply and pay online: Fill out and submit this membership application online. After you click Submit Form, you will automatically go to PayPal. Follow the instructions from there.
  • To apply online and pay by Visa or MasterCard over the phone: Fill out and submit this membership application online and then call our offices at 1-877-503-0806 (toll free).
  • To mail your application and pay by check, money order, or purchase order: Download and fill out this PDF version of the membership application. Make your check, money order, or purchase order payable to the National Tuberculosis Controllers Association and mail it along with your completed membership application to the National Tuberculosis Controllers Association, 2452 Spring Road SE, Smyrna, GA 30080-3828.
